What to bring for your appointment?
1. For timely service, please arrive at least 15 minutes prior to appointment time.
2. Please bring your insurance card and a photo ID.
3. If you are currently taking any medications, please bring a list of the same (or bring the medications itself).
4. If the patient is a child and/or needs a physical for school/college, please bring with you the Immunization card.
Please note that if you do not have insurance, you will be required to bring other forms payment with you when you come in for your appointment.
Visiting us for the first time?
In order to help us in preparing for your first visit, please print and complete the following forms in advance of your appointment:

Insurance & Payments:
Quality Care Family Practice participates with Medicare, Medi-Cal, IEHP, Prime Care of Corona, and most major PPO health insurance plans. If you have medical insurance, please provide that information to our staff when you check in for your appointment.
Please be advised that different insurance companies may cover different services, and some services you assume to be covered may not actually be covered. While we will gladly assist you in determining whether specific services and/or procedures are covered by your insurance plan, we strongly encourage you to contact your insurance provider for verification.
Please have your insurance card with you at the time of your visit. Regardless of whether we help you file your claim or not, you are responsible for the co-pay/deductible/co-insurance at the time of visit. If we are not a participating provider for your insurance, if you are not insured or if we do not file for your claims with your insurance company, you will be responsible for the entire charge at the time of service.
